The Québec Ombudsman received reports of major deficiencies in the quality of the care and services provided to the residents of a residential centre in Abitibi, primarily regarding basic care, nursing care, means of control, reprisals and harassment, and a nurse’s attitude and behaviour.

In 2014-15, the Parliamentary and Health Service Ombudsman carried out 4,000 investigations – ten times more than were completed in 2012-13. Rebecca Marsh, Executive Director of Operations and Investigations, explains the use of feedback to improve the service that people are expecting.

On December the 10th, the Office of the Ombudsman held a press conference to announce the results of two direct investigation reports.

Nearly three-quarters of hospital investigations into complaints about avoidable harm and death claimed there were no failings in the care given, despite the Parliamentary and Health Service Ombudsman’s investigations of the same incidents uncovering serious failings.

The Western Australian Ombudsman has tabled his 2014-15 Annual Report in Parliament. The report outlines the work of the office of the Ombudsman for the year, including investigating complaints, reviews of child deaths and family and domestic violence fatalities, own motion investigations and monitoring and inspection functions.

Process must not come before the rights of the individual when it comes to the reporting of abuse in the disability sector, said Victorian Ombudsman Deborah Glass. Tabling the second phase report, Reporting and investigation of allegations of abuse in the disability sector, Ms Glass said the system should support the people, not the other way around.

This summary report outlines relevant international and European standards and reviews national legislation and policies addressing violence against children with disabilities. The summary also explores the extent and different causes, settings and forms of such violence, and presents measures and initiatives to prevent it.
